Festoons or Malar Mounds
Drooping eyelid skin. Bags under the eyes. Puffy skin on the cheekbone. This condition is known as Festoons, or Malar Mounds.
A misdiagnosis of your eye bag condition could result in the wrong solution.
Although a blepharoplasty (see below) will rejuvenate the appearance of the eyelids for many patients, individuals with festoons, the condition characterized by puffy skin on the cheekbone just below the eyelid, requires additional treatment. If you have these, your eyes can actually look worse and more noticeable after undergoing a lower eyelid blepharoplasty or facelift. Once the fullness of the lower eyelid is removed, the problematic area caused by the festoon is highlighted, and becomes more obvious.
What are the typical options for treating festoons?
Festoons or malar mounds are a very difficult cosmetic problem to solve. In rare cases, festoons can be large enough to block an individual’s lower visual field. The most common options for treating festoons include:
- Extensive incisional procedures, which offer less-than-optimal results
- Direct excision of the festoons with unacceptable scars
- Medical therapy with marginal results

Upper and/or Lower Blepharoplasty or laser eyelid surgery

Blepharoplasty, also referred to as a “mini-facelift,” has become one of the most popular cosmetic procedures for both men and women suffering from drooping upper and/or lower eyelid skin due to its high level of patient satisfaction.
Blepharoplasty or laser eyelid surgery can provide the following benefits:

What to Expect

ConsultationDr. Scheiner will evaluate the condition and health of your eyes. He will also discuss with you the specifics regarding your vision, including tear production, use of contact lenses, use of medications and personal expectations. This information, along with other factors such as age, skin-type and ethnic background, will allow both you and the doctor to come to a mutual decision regarding your treatment.
Dr. Scheiner will also walk you through the surgical technique, amount of surgery and type of anesthesia he will use, and he will take measurements and “before” pictures to help him plan his treatment options. You will have the opportunity to see before and after pictures of other patients who have had the procedure. Additionally, you will watch an informative video, which discusses the procedure(s) and answers many common questions.
The ProcedureThis outpatient procedure is performed with local anesthesia in about one hour by Dr. Scheiner in our surgery center, which is located directly behind the Tampa Eye Clinic. You are allowed to return home immediately following surgery.
Dr. Scheiner uses a laser beam to make a small incision, sometimes from inside the eyelid. Laser eyelid surgery actually cauterizes (seals) blood vessels with less bruising, bleeding, discoloration, swelling, and a quicker recovery than conventional eyelid surgery. The fatty deposits are removed and the incisions are closed using tiny sutures. The sutures are removed in about ten days.
RecoveryYour post-operative discomfort should be minimal. Since the laser seals off blood vessels and nerve endings, you should experience much less post-operative discomfort or bruising. You can use Tylenol to minimize any discomfort. You can expect your eyelids to be puffy and swollen for several days following surgery. Cold compresses used for the first 48 hours can help minimize swelling. After 7-10 days the fine stitches will be removed.
Your normal activities can usually be resumed in just 3 to 7 days. The resulting scars are very fine and hidden in the natural crease of the upper eyelids.
