Our goal is to provide clear expectations regarding consultation fees, surgical deposits, payment requirements, pricing, cancellations, rescheduling, and post-surgical refinements. 

Please review this policy carefully. Our team is available to answer any questions regarding these terms before you proceed with scheduling your surgery. 

Physician Consultation Fees 

• In-Person Consultation: $250, non-refundable 

• Virtual Consultation: $100, non-refundable 

Consultation fees may be applied toward surgery, in-office procedures, or treatments performed within 12 months of your consultation. 

Appointments canceled with less than 48 hours’ notice or missed appointments may be subject to a cancellation or no-show fee. 

Surgical Deposits  

A non-refundable surgical deposit is required at the time of scheduling to reserve your surgery date: 

• 10% deposit for surgeries scheduled within 90 days 

• 20% deposit for surgeries scheduled more than 90 days in advance 

Your surgical deposit: 

• Is applied toward your total surgical balance 

• Is non-refundable 

• Reserves your agreed-upon surgery date, subject to the terms of this policy 

• Establishes price protection for your originally scheduled surgery date, subject to the pricing, surgical-plan, cancellation, and rescheduling terms outlined below 

Surgical Pricing & Price Protection 

Your surgical pricing is based on the procedures included in your rejuvenation plan and the practice’s fees in effect when you schedule surgery, sign your financial agreement, and pay the required surgical deposit. 

Once these requirements have been completed, your quoted surgeon’s fee is protected for your originally scheduled surgery date, subject to the terms of this policy. 

If you elect to reschedule your surgery to a date more than six (6) months after your originally scheduled surgery date, your surgical plan will be reviewed, and current surgical pricing will apply. 

If current pricing is higher than your original surgical estimate, you will be responsible for the difference. An updated estimate will be provided for your review, and any additional balance will be due according to the practice’s standard payment requirements. 

Final Payment & Surgery Date Protection 

All surgical fees owed to the practice must be paid in full no later than four (4) weeks prior to your scheduled surgery date. 

If full payment is not received by this deadline, your surgery date may be released and removed from the calendar. 

Surgeries that remain unpaid within fourteen (14) days of the scheduled surgery date will be canceled automatically. 

These requirements allow us to appropriately reserve operating room time, anesthesia, staff, and other resources necessary for your care. 

Surgeon’s Fees 

Your surgeon’s fee includes: 

• Pre-operative visits related to your scheduled surgery 

• Routine post-operative visits with Dr. Scheiner or designated medical staff related to your scheduled surgery 

The surgeon’s fee does not include: 

• Laboratory testing, EKGs, or imaging 

• Prescriptions 

• Medical clearance 

• Pathology 

• Hospital or emergency room services 

• Additional medications 

• Medical transportation 

• Unforeseen medical expenses related to surgery 

All clinical fees not otherwise included in your surgical agreement are due at the time of service. 

Surgery Center & Anesthesia Fees 

Surgery center and anesthesia fees are estimates based on the anticipated length of time for your procedure and surgical location. 

Tampa Eye Surgery Center 

Patients pay the facility fee directly to the surgery center and the anesthesia fee directly to the anesthesia provider prior to surgery. 

Center for Advanced Surgical Specialists (CASS) 

Our office collects applicable surgery center and anesthesia fees on behalf of the surgery center and anesthesia group four weeks prior to surgery. 

Final facility and anesthesia fees may be higher or lower than estimated depending on actual operative time and other applicable charges. 

Cancellation Policy 

We understand that circumstances may arise that require you to cancel surgery. Because your surgery date reserves Dr. Scheiner’s time, operating room availability, anesthesia, staff, and other resources, the following cancellation policy applies: 

• All canceled surgeries are subject to forfeiture of the non-refundable surgical deposit. 

• If surgery is canceled within fourteen (14) days of your scheduled surgery date, 50% of the surgeon’s fees will be forfeited. 

• Cancellations due to documented medical reasons will be reviewed on a case-by-case basis. 

Any exception to the standard cancellation policy must be approved by practice leadership. 

Rescheduling Policy 

We understand that circumstances may arise that require you to change your scheduled surgery date. Because surgery dates require the reservation of Dr. Scheiner’s time, operating room availability, anesthesia, staff, and other resources, the following rescheduling policy applies: 

• Your first surgery date change may be made at no charge if the change is requested more than 28 days prior to your scheduled surgery date and your new surgery date is within six (6) months of your originally scheduled surgery date. 

• If surgery is rescheduled within 28 days of your scheduled surgery date, a $1,000 non-refundable rescheduling fee is required to reserve a new surgery date. 

• If your new surgery date is more than six (6) months after your originally scheduled surgery date, a $1,000 non-refundable rescheduling fee is required to reserve a new surgery date. 

• Each additional surgery date change after the first requires a $1,000 non-refundable rescheduling fee to secure your new surgery date, regardless of the amount of notice provided.  

Any additional fees or balances resulting from rescheduling must be paid according to the practice’s standard payment requirements before the new surgery date is confirmed. 

Changes to Your Surgical Plan & Estimate 

Your surgical estimate reflects the procedures and products included in your rejuvenation plan at the time of booking. 

If your surgical plan changes: 

• Procedures added, removed, or modified will result in your estimate being updated accordingly. 

• Newly added or modified procedures will be priced according to the fees in effect when your revised rejuvenation plan is created. 

• Package pricing applies only when all procedures included in the applicable package are performed together. 

• Removing or changing a procedure may result in package pricing being recalculated. 

• Post-operative products and materials incorporated into your surgical plan to support recovery, healing, and optimal surgical outcomes will be reflected in your surgical estimate. 

Any revised surgical plan or estimate will be reviewed with you prior to surgery. 

Post-Surgical Refinement Policy 

Our Commitment to Your Results 

Dr. Scheiner is committed to delivering the optimal surgical outcome discussed during your consultation. Healing and tissue response vary from patient to patient, and in some cases, a refinement procedure may be recommended to enhance or optimize your final result. 

Refinement Evaluation 

Refinements are not routine and are determined solely by Dr. Scheiner based on medical judgment and clinical evaluation. 

• The need for a refinement may be identified during post-operative visits once healing has stabilized. 

• Refinements are intended to improve contour, symmetry, or healing-related changes. 

• Refinements are not intended to alter the original surgical plan or aesthetic goals. 

Eligible Refinements Within the First 12 Months 

If Dr. Scheiner determines that a refinement is clinically appropriate and eligible under this policy, no surgeon’s fee will be charged for the refinement when performed within twelve (12) months of your original surgery. 

The waiver applies to the surgeon’s fee only. Other costs associated with the refinement remain the patient’s responsibility as outlined below. 

Facility, Anesthesia & Other Refinement Costs 

Even when the surgeon’s fee is waived, you remain responsible for applicable costs associated with your refinement, including but not limited to: 

• Operating room or surgical facility fees 

• Anesthesia fees 

• Supplies 

• Medications 

• Products required for the refinement 

These fees vary based on the complexity, duration, and location of the procedure and must be paid in full before scheduling a refinement requiring operating room time or anesthesia. 

Office-Based Refinements 

Minor refinements performed in the office under local anesthesia may incur a procedural or supply fee. 

Any applicable fees will be reviewed with you in advance. 

Refinement Exclusions & Limitations 

Refinement coverage does not apply to: 

• Requests based on changes in personal aesthetic preference 

• New concerns unrelated to the original surgery 

• Weight changes, pregnancy, aging, or lifestyle-related changes 

• Non-compliance with post-operative instructions 

• Complications or conditions unrelated to the original procedure 

Refinements are not guaranteed and are not a substitute for revision surgery outside the intent of the original surgical procedure. 

Timing & Healing Considerations 

Refinements are typically not considered until adequate healing has occurred, which may take several months following surgery. 

Dr. Scheiner will determine the appropriate timing based on your healing progress and safety. 

Patient Responsibilities Following Surgery 

To remain eligible for consideration of a refinement, you are expected to: 

• Attend scheduled post-operative visits 

• Follow all post-operative instructions 

• Maintain realistic expectations regarding surgical outcomes 

Failure to follow post-operative instructions or recommended follow-up care may affect your eligibility for a refinement. 

Payment & Financing 

Accepted forms of payment include: 

• Visa, Mastercard, American Express, and Discover 

• Cash 

• Personal check 

• Money order 

• Wire transfer 

• ACH payment 

• CareCredit 

• PatientFi 

Financing approval, terms, interest rates, and payment requirements are determined by the applicable third-party financing provider. 

Insurance 

Reset Cosmetic Surgery, LLC is a self-pay concierge practice. 

• We do not accept insurance. 

• We do not submit insurance claims. 

• Patients are responsible for payment in full according to the terms of this policy and their applicable surgical agreement. 

Patient Acknowledgment 

By proceeding with surgery, you acknowledge that you understand and agree to this Post-Surgical Refinement Policy. This policy is intended to support patient satisfaction while maintaining safe, ethical, and medically appropriate surgical care. 

Patients review and sign this policy electronically through their Simplast patient portal as part of the surgical consultation and scheduling process.
