Consultation Fee

There is a non-refundable $250 in-person consultation fee or $100 virtual consultation fee with Dr Scheiner. This fee can be applied toward surgery, in-office procedure or treatment. Cancellation or No Show fees may apply if cancelled less than 48 hours.

Surgery Deposit

All surgery deposits are non-refundable and due at the time of scheduling surgery. A 10% non-refundable deposit is required to secure a surgery date within 90 days or a 20% non-refundable deposit is required to secure surgery scheduled greater than 90 days. Your deposit will be applied to the total amount due. All final payments are due four weeks prior to surgery.  As a courtesy, the deposit may be used towards non-surgical treatments if the surgery is cancelled.

Surgeon’s Fee

The surgeon’s fee entitles the patient to all pre and post-operative visits with Dr. Scheiner. Final payment is due four weeks prior to surgery. The patient is responsible for any and all additional fees necessary prior to or after surgery including but not limited to: labs, EKG, pre-operative testing or imaging, prescriptions, medical clearance and pathology etc. In addition, any and all unforeseen expenses related to surgery including but not limited to: emergency room/hospital fees, additional medications and medical transport are not covered under this agreement.

All clinical fees are due at time of service.

Surgery Center and Anesthesia Fees

The estimated surgery center and anesthesia fees are provided during the time of scheduling surgery based on the date and location selected. For surgeries performed at Tampa Eye Surgery Center, you will pay the surgery center facility fee directly to the surgery center and the anesthesia fee to the Anesthesia group, prior to surgery. For surgeries performed at Tampa Bay Surgery Center, our office collects these fees on behalf of TBSC Surgery Center and Magnolia Anesthesia one month prior to surgery. These fees are finalized after surgery and may be either higher or lower than the original estimate depending on the final operative time.

Cancellation Policy

All cancelled surgeries are subject to a Non-Refundable deposit.

Rescheduling Policy

One change of date may occur four weeks prior to the surgery date without incurring a fee. Additional date changes or changes with less than four weeks notice are subject to a change fee, per occurrence.

Insurance

Reset Cosmetic Surgery, LLC dba Adam Scheiner, MD is a complete self pay concierge practice.  The company does not accept insurance plans or file claims on behalf of patients. Patients are responsible for payment in full.
