Consultation Fees
- In-Person Consultation: $250 (non-refundable)
- Virtual Consultation: $100 (non-refundable)
Consultation fees may be applied toward surgery, in-office procedures, or treatments performed within 12 months.
Appointments canceled with less than 48 hours notice or missed appointments are subject to a cancellation/no-show fee.
Surgical Deposits
A non-refundable surgical deposit is required at the time of scheduling to reserve your surgery date:
- 10% deposit for surgeries scheduled within 90 days
- 20% deposit for surgeries scheduled more than 90 days out
Your deposit:
- Is applied toward your total surgical balance
- Is non-refundable
Final Payment & Surgery Date Protection
- All surgical fees must be paid in full no later than four (4) weeks prior to surgery.
- If full payment is not received by this deadline, your surgery date may be released and removed from the calendar.
- Surgeries not paid in full within fourteen (14) days of the scheduled date will be canceled automatically.
This policy allows us to appropriately reserve operating room time, anesthesia, and staff for your care.
Surgeon’s Fees
The surgeon’s fee includes:
- All pre-operative and post-operative visits with Dr. Scheiner or medical staff related to your scheduled surgery
The surgeon’s fee does not include:
- Laboratory testing, EKGs, imaging
- Prescriptions
- Medical clearance
- Pathology
- Hospital or emergency room services
- Additional medications or medical transport
- Any unforeseen medical expenses related to surgery
All clinical fees are due at the time of service.
Surgery Center & Anesthesia Fees
Surgery center and anesthesia fees are estimates based on procedure length and location.
- Tampa Eye Surgery Center:
Patients pay the facility fee directly to the surgery center and the anesthesia fee directly to the anesthesia provider prior to surgery. - Tampa Bay Surgery Center:
Our office collects these fees on behalf of the surgery center and anesthesia group four weeks prior to surgery.
Final fees may be higher or lower than the estimate depending on actual operative time.
Cancellation Policy
- All canceled surgeries are subject to forfeiture of the non-refundable deposit.
- If surgery is cancelled within 14 days of scheduled surgery date, 50% of the surgeon fees are forfeited.
Cancellations due to medical reasons require documentation and are reviewed on a case-by-case basis.
Rescheduling Policy
- One surgery date change may be made more than six (6) weeks prior to surgery at no charge.
- If surgery is rescheduled within 28 days of the scheduled date, a $1,000 non-refundable rescheduling fee is required to reserve a new surgery date.
- If a surgery is rescheduled a second time within six (6) months, an additional $1,000 non-refundable deposit will be required to secure a new date.
These policies exist to protect operating room availability and staff scheduling.
Estimate Changes & Surgical Plans
Surgical estimates reflect the planned procedures and include products discussed at the time of booking.
- If the surgical plan changes (procedures added or removed), pricing will be adjusted accordingly.
- Package pricing applies only when all planned procedures are performed together.
- Surgical estimates will include post-operative products and materials that are incorporated into the surgical plan to support recovery, healing, and optimal surgical outcomes.
Payment & Financing
Accepted forms of payment include:
- Visa, Mastercard, American Express, Discover
- Cash, personal check, money order, wire transfer, ACH payment
- CareCredit and PatientFi
Insurance
Reset Cosmetic Surgery, LLC is a self-pay concierge practice.
- We do not accept insurance
- We do not submit insurance claims
- Patients are responsible for payment in full
